Our Partners

Our work relies heavily on our community partners who volunteer their time and resources to “Make Change Happen”. Healthy Roanoke Valley provides an on-going forum for almost 160 volunteer members to step outside organizational boundaries and share leadership, expertise, and resources to activate a common set of shared goals for community improvement in an environment that offers mutual respect, trust and understanding while nurturing collaborative relationships. Our partners are a diverse group of cross-sector stakeholders representing health, education, housing, safety, income, government and access to healthy foods and physical activity.

Healthy Roanoke Valley is closely aligned with the Partnership for a Liveable Roanoke Valley, a program of the Roanoke-Alleghany Regional Commission. The Partnership created the “Liveable Roanoke Valley” plan to promote economic opportunity and quality of life in the Roanoke Valley and adopted Healthy Roanoke Valley’s strategies in their regional approach to health.

Since October 2012, volunteers have provided over 3,800 hours of service valued at $86,081.

Coordination of Care Action Team

We have been working toward a community-based care coordination system that connects residents to resources available in the community and helps these residents enter and navigate through the system. In fall 2016, we launched the HRV Pathways HUB. The current pilot, being implemented with Bradley Free Clinic, Carilion Clinic, and New Horizons Healthcare, is enrolling clients with chronic disease and connecting them with supports throughout the community.

Primary Care Action Team

We serve in an advisory and supportive capacity to Healthy Roanoke Valley initiatives that result in improved access to comprehensive primary care, coordination of care, and a culture of wellness.

Oral Health Action Team

We work to build awareness and understanding of oral health issues in the community toward the goal of oral health for all. We are working to increase the knowledge and value of oral health in our community and to strengthen and expand the oral health safety net to make affordable services more accessible.

Mental Health & Substance Abuse Action Team

We work to increase awareness and knowledge of mental health and substance use issues by educating the Roanoke Valley community.

Wellness Action Team

We work to create a culture of wellness in the Roanoke Valley by improving access to healthy and affordable foods; providing opportunities for safe physical activity and active transportation; and reducing risky behaviors in youth and young adults.
